Physicals:
All student athletes will need to have a current physical for the upcoming shcool year. You may go to your own doctor or you may go to Dr. Terry Orme for a flat fee of $25. You may pick up a physical form at the front desk of SBA, or you may go to www.tssaa.org and click on forms then print the TSSAA Preparticipation Medical Evaluation Froms part 1 & 2.
Dr. Orme is located at 8644 East Brainerd Rd. Chatt. TN 37421 which is at the corner of East Brainerd Rd. and Morris Hill Rd. in the Hurricane Creek Plaza (Urgent Care Family Medicine). His phone # is 423-648-6677. You may just drop in or you may call for an appointment. Please be sure to identify yourself as a Silverdale Baptist Academy student athlete.
